New Mexico Heritage Health Index Survey
Thanks to a grant from the Institute of Museum and Library Services (IMLS), the New Mexico Association of Museums and the Conservation Department, Museum Resources, DCA is able to conduct the New Mexico Heritage Health Index Survey of the collection holdings of museums, libraries and archives within New Mexico. This survey is being conducted in partnership with the New Mexico State Records Center and Archives and the New Mexico State Library.
Why should my institution complete the survey?
Collecting institutions in New Mexico are responsible for the care and preservation of the unique and irreplaceable items they hold in public trust. It is important to be aware of the conditions of the collections and issues involving their long-term care. The New Mexico Health Heritage Index Survey will allow institutions to identify some of these issues and allow a common voice to be heard.
This survey will allow the compilation of a report on the health of collections throughout the state of New Mexico and the creation of a plan for addressing some of the most critical collection conservation needs. The plan will be submitted to IMLS in order to compete for $250,000 in future funding which will allow implementation of the plan, benefiting all participating institutions.
You must take part in the survey portion of the process in order for your institution to be included in the implementation stage of the conservation plan.
How do I get a copy of the survey?
Download a copy of the survey along with instructions for its completion. Please forward the survey to the appropriate
person(s) in your institution to complete. Information gathered through the survey will be used
for statistical purposes only and the individual responses will be kept
confidential.
The questions in the survey are in the form of an interactive PDF and can be answered directly on your computer screen. Open the form and then save it on your computer with a unique name. You will be able to fill in the survey at your own pace, saving your changes after each session and picking up again where you left off. When you have completed the survey, save and/or print a copy for your records.
The completed form is easily submitted electronically by clicking on the “SUBMIT THIS COMPLETED FORM” button at the bottom of page 26. If you choose, you can return a printed copy by mail to: Kristi Catanach, HHI, Conservation Department, 725 Camino Lejo, Santa Fe, NM 87505.
